Continuity of NHS medical care - change of address/contact details

Please read the notice below if you are registered with a GP in Edinburgh.

Many thanks to our registered patients who have provided us with new addresses/updated contact details over the last few months.  If however you have moved/changed details and have not notified us of a change of address/contact details then please do so as soon as possible as per the details below.

In order to ensure continuity of medical care it is necessary to notify your Edinburgh general medical practice of any change of address/contact details.  Failure to do so may mean that you become ineligible for health care should you need it.  If your GP tries to contact you and finds that you no longer reside at a particular address then you may be removed from the practice list.  Please make sure you inform your practice of any change in address/contact details by contacting your practice.

Details required are your full name, date of birth, previous and new address. If you have changed your email or telephone number, please also let us have this information.

For those students who are registered with University Health Service your change of address/contact details can be notified by email, telephone, calling in or writing to us as soon as you move.  You can call the University Health Service on 0131 650 2777.  You can also email us on Univhealthserv@nhslothian.scot.nhs.uk.  If you are registered at another GP practice then please contact them directly.

If you are moving away from Edinburgh permanently then please let your GP know the date you will be moving, and whether you are going abroad or moving within the UK so that records can be adjusted.  If you remain within the UK then you should register with a GP in your new location as soon as possible.
