To start a new piece of radioactive work at the University a suitable and sufficient Radiation Risk Assessment must be completed.

The previous method of approaching risk assessments, which included a generic risk assessment and a PSoW, is currently being phased out. For any new radiation work please visit the Risk Assessment page to complete the new style Radiation Risk Assessment.

Proposed Scheme of Work templates

A Proposed Scheme of Work form needs to be submitted and approved before each individual starts new work with radioactivity. Please select the appropriate form from the list below, if you are not sure consult with your RPS for advice. These PSoW forms should be used in combination with the Generic Risk Assessments produced by the RPU.

These forms can be filled in and submitted electronically (submit button opens Outlook). You will need to click "enable macros" when opening these forms. Please note that the submit button does not work with Apple devices.

These forms can be downloaded and filled in either electronically or by hand. Once completed, and signed by the RPS, these forms can be emailed or posted to the University RPA.

Old forms

Please note that as of 31st December 2018 the old version of the Unsealed Radionuclide PSoW will no longer be accepted by the RPU. The new form, found above and first introduced in December 2017, will be accepted by either electronic or paper submission.
