Accident and Incident Reporting Form
Use this form to report accidents, incidents and dangerous occurrences,'near misses', and instances of known or suspected occupational ill health to the University Health & Safety Department. Accident or Incident Report Form
NOTES: All fires and other dangerous occurrences, accidents and 'near misses', and instances of known or suspected occupational ill health, must be reported to the School Health & Safety Manager as soon as possible after the event. No accident or incident should be considered too trivial to report.
A report must be sent to the Director of Health and Safety within seven days of the event. The report form may be completed by the individual or the Health & Safety Manager, but a copy must always be forwarded to the Health & Safety Manager. This happens automatically if the contact details for the Health & Safety Manager are given on the form, otherwise you must print and forward a paper copy. Completed forms are filed securely to preserve confidentiality.
Any accident, incident, or occurrence of suspected occupational ill health, which results in absence from work must be reported to the University's Director of Health & Safety on the fourth day of absence or on return to work, whichever is the sooner, using the web form.
Accidents and incidents of a serious nature must be notified immediately by telephone (51 4255) to the University Health & Safety Department. The University Health & Safety Department report the accident or incident to the Health & Safety Executive if this is necessary.
