Accident and Incident Reporting Form

Use this form to report accidents, incidents and dangerous occurrences,'near misses', and instances of known or suspected occupational ill health to the University Health & Safety Department. Accident or Incident Report Form

NOTES: All fires and other dangerous occurrences, accidents and 'near misses', and instances of known or suspected occupational ill health, must be reported to the School Health & Safety Manager as soon as possible after the event. No accident or incident should be considered too trivial to report.

  • A report must be sent to the Director of Health and Safety within seven days of the event. The report form may be completed by the individual or the Health & Safety Manager, but a copy must always be forwarded to the Health & Safety Manager. This happens automatically if the contact details for the Health & Safety Manager are given on the form, otherwise you must print and forward a paper copy. Completed forms are filed securely to preserve confidentiality.

     

  • Any accident, incident, or occurrence of suspected occupational ill health, which results in absence from work must be reported to the University's Director of Health & Safety on the fourth day of absence or on return to work, whichever is the sooner, using the web form.

     

  • Accidents and incidents of a serious nature must be notified immediately by telephone (51 4255) to the University Health & Safety Department. The University Health & Safety Department report the accident or incident to the Health & Safety Executive if this is necessary.
