When you arrive at the Student Counselling Service for your initial appointment, you will be asked to sign our Registration Form giving us permission to keep confidential case-notes.
We ask permission to keep confidential case-notes for five years in order to conform to the Data Protection Act. These notes are kept securely and are only accessible to Counselling Service staff except in very exceptional circumstances (as outlined in our Code of Practice).
We will also ask you to complete our Clinical Outcomes in Routine Evaluation (CORE) self evaluation questionnaire where you are required to quickly tick boxes in response to questions about how you have been feeling in the week leading up to your initial appointment. If you attend for counselling, we will ask you to complete the same questionnaire after your last counselling session.
You will be given a copy of our Code of Practice when you come to the Service and you will be asked to sign a form to agree to notes being kept.
The Initial Appointment may take up to 50 minutes
