What documentation should be completed?

There are a number of standard forms which may be used.

Current University P&DR forms

Guidance and forms are available from Human Resources.

Individual School forms

Several schools within the college have developed their own P&DR forms which continue to be used.

Reviewers should contact their own line manager for guidance as to the most appropriate P&DR form by their staff.

Local forms can be found on a School intranet, wiki or website.

College - New forms

These new template forms have been developed on the basis that, while all reviews have the same underpinning principles applied to the process, there are different performance measures and development needs which require consideration when reviewing a diverse and multilayered workforce.

The forms can be completed electronically or by hand and may be adapted as required. Regardless of which set of forms are used, the substance should remain the same since this reflects the core principles set out in the guidance notes. Also, It should be noted that whilst the forms may be useful these should not detract from the quality of the discussion.

Please note that included with College documentation are pro-forma for recording individual learning and development plans and also for collating group learning and development plans which can be used to inform any staff development planning process.

The following stages should be undertaken regardless of the form of choice:
